Reporting an incident

This goes straight to our dispatch team and becomes part of the permanent record for this venue. Anyone can file one — you don't need to be signed in as a response officer.

  • Be specific — exact times, names if known, what you directly saw.
  • Attach a photo if it's safe to do so — it strengthens the record.
  • File it as soon as you can — details fade fast from memory.
  • You'll get a reference number — save it in case anyone follows up.
In an emergency, call 999 first. This form is for reporting and record-keeping, not for getting immediate help.
Use this form to report any incident — whether you're one of our mobile response units or on-site venue staff.
This is for something that's already happened — an assault, theft, medical issue, anything that's taken place. Nothing happened yet, just a hazard or something unsafe? Report a site risk instead. Had to physically restrain someone? Use of force report instead.
